Sumner Hospice
Sumner Hospice admission criteria is as follows:
- The patient must have a diagnosis of a terminal illness with a limited life expectancy.
- Aggressive treatment for the terminal diagnosis has been discontinued.
- There is a physician's order for hospice care.
- The patient and/or caregiver understands and desires hospice care and accepts the goals of comfort, pain and symptom management, and peaceful death with dignity.
- The patient and/or caregiver will identify a capable caregiver responsible for 24-hour care when necessary.
Discuss home care options with your physician, who will make a referral to the agency of your choice. A social worker or discharge planner in a healthcare facility may also assist with the process. Hospice is available with a physician's order to patients with:
- Medicare/Healthspring
- Private Insurance
- Worker's Compensation
- Private Pay
